Comprehensive residential services for developmentally disabled children and adults
Our chief Executive Officer, Lisa LaTray, has over 23 years experience in the
developmental disability field. She has hands on and management experience
ranging from direct care work at a state run institution to running a statewide
developmental disabilities agency. Lisa has developed services out of state
along with state and federal public policy experience.
Teresa Siler, Office/Program Manager, she is very organized and believes in the
mission of Phoenix Residential Services. Teresa is very supportive of our
Profess ional HTS staff and is always ready to answer questions and help. Her
great nature is very calming and all who are around her find her a pleasure to
work with.

Valorie Juarez is taking Janie’s position as Program Director. Valorie has over 15
years experience working in the field. She has an excellent reputation for
getting anyone who has the desire to work a job in the community that is real
and pays good money. Phoenix is pleased to have Valorie join our team. We
hope that her enthusiasm for finding jobs will help our customers and staff find
many wonderful things to do in the community.
Phoenix Residential Services is entering its fourth year as a Community Service
Provider through a contract with the Oklahoma Department of Human Services
Developmental Disabilities Services Division. We provide HTS, DLS, In-Home
Supports waiver Services and Adult Companion Services. We have really
enjoyed watching people who receive our services grow and learn. Some of our
first customers have joined our entrepreneurial spirit and became the first
people in Oklahoma to try Self Directed Services by being part of the SDS Pilot
Program. We are so proud of these parents who took the chance to make a
change for their children.
We specialize in periodic staffing services needed by parents who care for their
children at home and have the need for flexible services. We currently serve 78
people in the IHSW, Community Waiver and Supported Living Services. We
provide services in Tulsa, Creek, Osage, Washington, Payne, Mayes, Craig,
Rogers, and Pawnee counties. Our home office is in Sand Springs, but don’t let
that stop you from considering us. We work with you so you do not need to
drive here — we can come to you and a lot of our paperwork can be sent
through e-mail, faxes and regular mail.
